We are out of network with Managed Medicare plans.
Cash payment is accepted for skilled PT services for those without insurance or for wellness care
*** HSA/FSA may be used to pay for wellness care services

Why should I choose Arise Physical Therapy and Wellness?
When you choose Arise Physical Therapy and Wellness you will receive one-one- one care every single visit, provided by a licensed physical therapist. Misty Cozart-Evans, PT is the only therapist you will work with each visit. Misty has 15 years experience in treating a variety of conditions including neurological, cardiopulmonary, orthopedic diagnoses and more. She will develop a custom program tailored to meet your individual goals. Misty is a Christian and strongly believes in the power of one's faith in healing and achieving your personal goals. She will treat you like family!

What should I expect on my first visit?
I come to you! All visits are completed in the comfort of your own home. During the initial assessment consent forms will first be signed. Then, I will ask you questions and complete a full assessment of strength, endurance, balance, gait, transfers, home safety, etc as needed. The initial assessment sometimes requires 1 hour to 1.5 hours to complete. Based on the assessment findings a plan is developed to meet your individual goals. You can decide at this time if you would like to proceed with services. If you choose to proceed with care, subsequent visits can be scheduled at this time. Payment will be due prior to initiating each visit.
How often can I be seen?
For skilled physical therapy visits, your physician may order a specific number of visits. However, the number of visits can be flexible according to your skilled needs and budget, with the approval of your physician.
Wellness visits are typically completed weekly. However you may request visits to be completed quarterly, monthly, weekly, or multiple times a week at your request. We can customize your scheduled visits to meet your needs and budget.
Do I need a doctor's order?
A physician's order is required for skilled physical therapy treatment. However, wellness care does not require a physician's order.
